Nurse Practitioner (NP) or Physician Assistant (PA) – Kidney Transplant/Nephrology (OUTPATIENT)
Location: New York, NY
Schedule: Full-Time | Monday–Friday | Day Shift | Hybrid flexibility
Compensation: $135,000–$160,000 (based on experience)
We are seeking a Nurse Practitioner or Physician Assistant to join a nationally recognized outpatient Kidney Transplant program. This is an excellent opportunity to work alongside a multidisciplinary transplant team providing comprehensive care to kidney transplant recipients throughout their recovery and long-term follow-up.
This role combines direct patient care with care coordination, allowing providers to build lasting relationships with patients while practicing in a collaborative, fast-paced environment.
What You’ll Do
- Provide outpatient care for kidney transplant recipients and living donors.
- Perform patient assessments, histories, physical exams, and develop treatment plans.
- Collaborate closely with transplant nephrologists and the multidisciplinary care team.
- Manage medications, including immunosuppressive therapies and prior authorizations.
- Review and interpret laboratory and diagnostic results.
- Coordinate patient care, education, and follow-up both in person and through telephone/virtual communication.
- Participate in quality improvement initiatives, patient education, and clinical protocol development.
- Maintain accurate documentation within the electronic medical record.
Qualifications
- Licensed Nurse Practitioner or Physician Assistant eligible to practice in New York.
- Experience in transplant and/or nephrology preferred OR new graduates with prior inpatient RN experience (working independently and managing medically complex patients)
- Strong clinical judgment, critical thinking, and organizational skills.
- Ability to work independently while collaborating effectively within a multidisciplinary team.
- Excellent communication and patient education skills.
- BLS certification required. DEA certification required upon hire or within employer guidelines.
